What to Know Before You Come In
Paperwork for patients new to Perry Chiropractic is available online, and completing it ahead of time allows our chiropractors to have your health history accessible to them during your appointment. It’s also helpful if you can bring a list of any current medications you’re taking, as well as any recent X-rays.
Please bring your ID and insurance card with you to your appointment. We can even verify your insurance coverage before you come in, if you would like.
And finally, because our thorough exam includes testing your ranges of motion, we ask that you wear comfortable clothing that allows you to move freely, and no dresses.
Online Forms
Your First Visit
After our cheery front office staff greets you and assists with any questions or insurance issues, you’ll come back to meet with your chiropractor. He’ll listen closely as you tell him what’s been going on with your body and what your health goals are. Then he’ll take digital X-rays and conduct a thorough examination. In some instances, we may adjust on the first visit, but typically that will happen at your second appointment.
Your Second Visit
Your doctor will review your X-rays extensively at your second visit, correlating his findings with the pain or condition you’ve been experiencing. He’ll explain what’s causing the pain and lay out his recommendations for treatment. Before you consent to receive care, we will let you know exactly what your costs will be, including your deductible and copays, so you have all the information you need to make an informed decision. We never blindside patients with surprise fees.
We are in-network with many insurances, including Medicare and some Medicaid plans, and accept all major credit cards and Health Savings Accounts (HSAs).
